The Program Manual and Application Forms for New Crop, and Livestock Production can be downloaded below. There are three major categories of certification for Growers: Crop Production, Wild Crop Production and Livestock Production. There are also sub-categories of both Crop & Livestock Production.
Make sure you read the category descriptions carefully when downloading forms. Call our office at (774) 872-5544 if you need any help! Forms are in both Microsoft Word (.DOC) and Adobe Reader (.PDF) format. NOTE: only one Fee Worksheet needs to be filled out, no matter how many categories of production you are certifying.

Maple Syrup Forms - There are two sets of maple syrup forms. The first set is for producers that are only certifying maple syrup production. The second set is for producers that want to certify maple syrup along with other crops.

LIVESTOCK PRODUCTION FORMS
There are three (3) different sets of forms for Livestock Production:
If you want to certify livestock in two or three different categories then you will need to fill out a set of forms for each category of production. NOTE: you only need to fill out the Dairy Crop Plan OR the Livestock Crop Plan, you don't have to fill out both of them.
Organic Dairy Application Forms (for certifying dairy animals) - DOWNLOAD all items:
Print out and submit the Dry Matter Intake Calculation Worksheet for each class of ruminant animals you are certifying. Use the Instructions and the Demand Tables for either Cattle or Goats to help you fill out the forms.

Organic Livestock Production Forms (for certifying ruminant and non-ruminant livestock for slaughter) - DOWNLOAD all items:

1) Dairy Production Forms are for certification of Dairy Animals and ruminant animals for slaughter that are culled from the dairy herd;
2) Livestock Production Forms are for the certification of ruminant and non-ruminant animals for slaughter;
3) Poultry Production Forms are for the certification of laying hens, broiler chickens, duck, geese, turkeys and other poultry.
