There are three major grower categories of certification: Crop ProductionWild Crop Production and Livestock Production. There are also sub-categories of both Crop & Livestock Production. The Program Manual and Application Forms for New Crop, and Livestock Production can be downloaded below. Make sure you read the category descriptions carefully when downloading forms. Forms are in both Microsoft Word (.DOC) and Adobe Reader (.PDF) format. NOTE: only one Fee Worksheet needs to be filled out, no matter how many categories of production you are certifying.

MAPLE SYRUP

Maple Syrup is one of the easiest crops to produce organically. Please make sure you submit your application for certification of maple syrup in late fall or early winter, so BOC can certify your operation before the year's sap run. There are two sets of maple syrup forms. The first set is for producers that are only certifying maple syrup production. The second set is for producers that want to certify maple syrup along with other crops.

WILD CROP PRODUCTION FORMS

These forms are for Wild Crops certification ONLY. See the NOP Wild Crop Guidance [ PDF ]  as to what constitutes as a wild crop or contact our office. DOWNLOAD all items

 

LIVESTOCK PRODUCTION FORMS

There are three separate sets of forms for Livestock Production:

  1. Dairy Production Forms: for dairy animals and slaughter animals culled from the dairy herd;
  2. Livestock Production Forms: for ruminant and non-ruminant animals for slaughter;
  3. Poultry Production Forms: for laying hens, broiler chickens, duck, geese, turkeys and other poultry.

If you wish to certify livestock in two or three different categories then you will need to fill out a set of forms for each category of production. NOTE: you only need to fill out the Dairy Crop Plan OR the Livestock Crop Plan, you don't have to fill out both of them.

DAIRY PRODUCTION - DOWNLOAD all items

Print out and submit the Dry Matter Intake Calculation Worksheet for each class of ruminant animals you are certifying. Use the Instructions and the Demand Tables for either Cattle or Goats to help you fill out the forms.

MULTIPLE TYPES OF LIVESTOCK FORM

Use this form if you are certifying multiple forms of livestock. This replaces the livestock form only. You still have to submit the other forms.
