Forms: Manual and Applications for New Farms

There are two major grower categories of certification: Crop Production and Livestock Production. There are also sub-categories of both crop & livestock production, including Poultry, Wild Crops, Mushrooms, Sprouts, and 100% Grass-Fed Certification. The Program Manual and application forms for new applicants can be downloaded below. Make sure you read the category descriptions carefully when downloading forms. Forms are in Microsoft Word (.docx) unless otherwise noted.

NOTE: Only one Fee Worksheet needs to be filled out, regardless of how many categories of production you are certifying with the exception of grass-fed certification. If you are seeking 100% Grass-Fed Certification, there is an additional fee and fee worksheet for that scope.

Crop Production Forms

VEGETABLE, FRUIT, PASTURE, SPROUTS, MICROGREENS, HEMP, GRAINS, NUTS, & OTHER CROPS – DOWNLOAD all items.

Livestock Production Forms

There are three separate sets of forms for Livestock Production:

  1. Livestock Production Forms: for all livestock production including dairy and slaughter stock and for livestock plus poultry production;
  2. Grass-Fed Certification Forms: for grass-fed dairy and grass-fed ruminant slaughter production;
  3. Poultry Production Forms: for poultry only production.

Submit the DMI Calculation Worksheet for each class of ruminant animals you are certifying. Use the Instructions to help you fill out the forms.

Supply Chain Additional Forms

Some suppliers and storage facilities that you might be using may claim to be exempt from being certified organic themselves. Please have them complete the Exempt Operation Affidavit so that you can submit it to Baystate Organic Certifiers as soon as possible. If you have any questions about whether any of the operations in your supply chain need this form, please contact Baystate.
